Blood Glucose Monitoring: The app allows patients to track their blood glucose levels throughout the day, with the ability to include comments about the insulin they are taking and also anything that may be relevant in regards to their glucose levels. These readings are able to be reviewed by the healthcare team so they can help monitor remotely.
2.) Diet and exercise tracking: The app includes a food diary and exercise log, allowing patients to track their daily intake and activity levels. The app allows patients to take pictures of their plates with the ability to make notes. Also you are able to set goals with this app to help keep patients on track.
3.) Communication with healthcare team: The app allows patients to communicate easily with their healthcare team, including the doctor, physician assistant, nurse practitioner, medical assistants, diabetes educator, weight loss coach and front desk staff. Patients can ask questions, share information, request refills and lab results and receive feedback and support.
4.) Education and resources: The app includes educational resources on diabetes management. We can also provide personalised tips and recommendations based on the patient's profile and activity.
