Diameter for PC

The Diameter is a free smartphone app for patients with type 2 diabetes. With this app you can keep track of your nutrition, activity and glucose values ​​in one environment. You can also set a personal goal in terms of nutrition or activity. Every week a digital coach discusses these goals and tries to help you achieve these goals. In addition, you can receive informative and motivational messages in the field of nutrition and activity for 10 weeks.

By filling in nutrition you get insights into your daily calories and carbohydrates intake. You can enter activities manually, but for extra insights into daily movement you can also link the Diameter with a Fitbit activity tracker. This gives you an overview of the movement during the day. In addition, you can manually enter your measured glucose values ​​and use the Diameter as a diary.

If you use the Diameter in ZGT hospital, it is possible to merge the lifestyle data with continuous glucose data, measured with the Freestyle Libre. In ZGT you can receive an overview that provides insights into the influence of activity and nutrition on glucose levels. Your internist or diabetes nurse can then use this to better treat your diabetes. You can use the insights yourself to prevent glucose peaks in the future.

For 10 weeks you can receive informative or motivational coaching messages twice a day. These messages are sent as a push notification. The coaching messages focus on the phases of behavioral change, in which all 4 phases of behavioral change are discussed during ten weeks. You can choose to receive messages about physical activity and / or nutrition.

With the Diameter, you have the option to set a weekly lifestyle goal and track progress towards achieving the goal. Three types of lifestyle goals can be set: a steps goal, a cycling goal or a nutritional goal. With the Diameter you can monitor whether you have reached your lifestyle goal. Step goals can be checked automatically when you also use the Fitbit. Monitoring of cycling goals and nutritional goals is based on self-reporting. For this you will receive a daily push notification to indicate whether you have reached your goal.

Finally, the Diameter offers you weekly exercises. With the feedback you get information about how often the lifestyle goals have been achieved, including a reflection on the reasons why the lifestyle goals have / have not been achieved. The exercises are based on behavior modification techniques that provide tools to support the achievement of lifestyle goals.
